Care Planner - SHAPE Team

We are seeking a Care Planner to join our SHAPE team and help achieve the best outcomes for the people we support at home and in the community. If you are passionate about person-centred care and enjoy working closely with clients and healthcare professionals, this could be the ideal role for you.\n\nWhat Will You Be Doing?\n\nAs a SHAPE Facilitator, you'll play a pivotal role in delivering strength-based care that promotes independence and well-being.
